When placing your order, Please fill in the Customized Pic Category, Number (Box) of your choosing.(ex: motivational (Mo001)
Fill in Patients First Name and First Initial of Last Name (Box).
Or if you have a personal picture you'd prefer to use, E-Mail it to me as an attachment.
The pictures are for the front of your organizer.
When sending e-mail, be sure to include the first name and the first initial of the last name for whom the organizer will belong to (ie: The Patient).
